New Patients

At House Dental Centre we are happy to accept new patients! We ask that all new patients arrive 15 minutes prior to their appointment time to fill out their chart that includes a patient’s past medical and dental history.

For your first appointment, we ask that you bring the following information to aid us in assisting you:

Your insurance information, including your policy number, group number, and insurance booklet if you have one.
A list of all medications that you’re currently taking, including the doses.
Your physician’s information, such as name and phone number, if you are currently under one’s care.

Please advise our reception team if you require any antibiotic premedication prior to dental treatment. This may be necessary if you have any heart conditions or if you have had joint replacement surgery.
